| All
permission slips will be verified the Tuesday prior to the camp
out. Any change of medication will require a new Class I
Medical Form to be completed. It is the
responsibility of the Scout's parent to inform the Troop of any
changes. A Scout may not attend the activity if the updated
form is not completed prior to departure. Any
medicine or prescriptions to be given to the Scout must be in the
original container with his name, dosage, and times. This
information must also be on the medical form for the Troop to
dispense. All prescriptions or medicine is to be turned into the
designated health and safety leader for the activity at
check in time. |

inhaler needs to be listed on the 3x5 card and the leaders in
charge notified.) You will need to put your Scout's
medications or non prescription medications in a zip look baggie
with the instructions written plainly on a 3x5 card with your
